Tools for Living Healthy with Diabetes Program
Enroll in the 6-month diabetes wellness program and get the tools you need to stay healthy. If you are new to diabetes or a veteran, this program will help you with the most up to date information and help you make a plan to stay healthy. You'll start with an individual health assessment, take part in a fun, interactive group class, and receive professional support and access to education for 6 months.
